SNM, or any person acting on their behalf, are simply recording the information that you give. They do not purport to diagnose any condition, or make any causal link between any medical symptoms or condition that you describe and the cause of these.
Your data will not be shared with any other agencies without your express permission.
Information submitted through our website or by another other means (including, but not limited to email, TXT, messenger, or phone) will be stored securely in a confidential database, in line with our obligations under the New Zealand government privacy legislation.
Access to your data is limited to only those with administrative login permission to the database, who can see, add or change that information on your behalf.
